I understand that tattoos are permanentand that removal, if desired in future, is difficult, costly, and may not fully restore my skin’s original appearance.

I understand the risks involved, including but not limited to infection, allergic reaction, scarring, colour fading or change over time, and variation from any reference design due to the nature of skin and freehand work.

I agree to follow the aftercare instructions provided by my artist, and understand that improper aftercare increases the risk of infection or poor healing, and is my own responsibility.

I confirm I am not currently under the influence of alcohol or drugs, and I understand HighSpire Tattoo reserves the right to refuse or reschedule treatment if there is any concern about my ability to give informed consent.
